Thank you to Mohammad Zareie for meeting me at the site on Wednesday. Attached is the field citations for violations I observed at the inspection.

There are three corrective actions that require immediate attention:

1. 1. Pump out the liquid in the tank and submit to me a fuel disposal receipt
2. 2. Submit an owner modification form (I left a paper copy with Mr. Zareie)
3. 3. Submit proof of financial responsibility

In addition, you have not paid annual tank fees since you bought the site on 3/31/2021. I’ve also attached a summary of the tank fees that you currently owe. The total is $1,770.

The temporary closure status of your tank expired on October 3, 2020. By July 29, you must submit either a 30 Day Notice to Decommission or a Return to Service application, with firm dates for when the work on the tanks will be performed.

The payment of the $1,400 penalty for the field citation is also due on 7/29/20204. If you choose to contest the field citation by not responding or paying the penalty, I will then refer the matter to DEQ’s Office of Compliance and Enforcement for formal enforcement.
