Attached is the Permit Form for sewer lateral repair and City Standard Drawings for the Street Cut, Lateral Repair & Sidewalk (if locate wire is not present, not required in the repair). Please submit the following information:

· Completed permit, both pages signed (Fee $150, with Street Cut)

· Business License Application ($70)

· Certificate of Liability Insurance – (naming the City for the $ amount of the repair work)

· Traffic Control plan per MUTCD standards
