I called Tiffany back this afternoon at 3:25 PM. She did not answer. I left a message confirming her call and thanking her for calling. I stated that we would like to see the letters and send them when she had a chance.
Attached are the two letters we received from Tiffany. Neither are related to the issues we had at the plant. Although the letter to Lyle in 2007 is similar, the cause of the alarm failure is not the same. The programming of our alarms are and were functioning at the time of the May 2015 event. The second letter relates to a calibration issue. I would not expect the alarm to call out if the calibration was out and reading within spec even though it was not. This can be confirmed by Jon Patrick next week. He will not be in this Friday.
