I have attached the form that the auditors like, but an email from the department head and/or city manager can also work. It is whatever your preference is.

My instructions from Dan are to make no changes to Jason Clifford for this payroll 02/26-03/10, payable 03/15. He said he is waiting on the job evaluation and further information from the attorneys to make a decision. Whatever you two decide can be retro-dated back if you determine that is necessary but it is too late to change anything for this pay period.
