Hi Jenn,

Attached are the completed 1200-C application, Land Use Compatibility Statement (LUCS), and an instructional document (fact sheet). Please review and provide signatures on the Application and LUCS. Along with the two signed documents the following items are required to be submitted to DEQ:

-Full-size (22” x 34”) Erosion & Sediment Control (ESC) Plans

-Electronic copies of signed Application and LUCS, and ESC plans

-Application fee of $1932.00; make check payable to Oregon DEQ

Application materials should be submitted to DEQ at this address:

Oregon Dept. of Environmental Quality

Northwest Region, Water Quality

Portland, OR 97232

We have a couple ways to handle the submittal to DEQ:

1. Return signed Application, LUCS & application fee (check) to me and MSA will submit the packet to DEQ.

2. I can send the full-size & electronic plans to you and the City can make the submittal.
