This is going to be an ODOT Maintenance District 2B issue. The probably was no riprap installed at the time of construction around the outlet to prohibit erosion failure.

Subject: Issue At 213/Meadow Dr
I wanted to report an issue I saw out in the field yesterday. The yellow lines here are the storm system at the west end of Meadow Drive. The red, circled area is where the storm outlet ought to be. The shoulder area of the road has crumbled into the ditch, presumably covering the outlet.
