This spring, DEQ's Northwest Region Office will be moving to a new location - the 700 Lloyd Building at 700 NE Multnomah St., Suite #600, Portland, OR 97232. The target date for operating at the new location is May 26th, 2015.

Hi Jennifer,
As a follow-up to our conversation this afternoon, the link to the form that needs updating can be found here: Wastewater System Operator Designation (the form is housed on the Oregon DEQ wastewater operator certification homepage).
I have attached the letter that informed Erik Moorhouse that his most recent application is closed, why it was closed, the remedy, and also that the facility can reference the attached letter to apply the $200 of unused exam fees towards a future operator certification application.
I recommend Mr. Moorhouse to submit a $50 post-exam application within the next two weeks. This is because he passed the exams in March of 2014, and these passed exam scores will only be eligible for certification for the next two weeks if he demonstrates through a post-exam application form that he has obtained the experience that he did not have at the time he took the exams.
