Hi Tony,

I know it has taken some time but I have attached the signed Service & Maintenance Rate Agreement.

Additionally, I noticed I do not have a Certificate of Liability Insurance in my file for Cherry City. Please provide a COLI naming the City of Molalla at the Certificate Holder. Our minimum COLI requirements are $1M Ea. Occurrence and $3M General Aggregate.
