Here's two examples given to me last year. I would have them contact Ellen and discuss their needs for the permit. I'm sure she can provide a blank permit and the process.

Tanner is on vacation and I am helping him with the holiday decorations. We realize that many municipalities have been hanging electrified decorations for many years. However, PGE wants to know, each year, who is hanging decorations and where. We also want evidence of the proper liability coverage as described in the application each year. Therefore we require a new application each year and not a copy of last year’s application. We also bill for electricity used for the decorations via this application. Molalla was not billed in 2012 or 2013 because no application was submitted.
