Jen,
The attachment lists me as having a level 1 treatment. I do not have a Level 1 treatment. I chose to skip the level 1 test to save the city a few bucks and submitted my application for my level 2 treatment test to DEQ on 1/2/15. I should hear back from them this next week as they have a 4-6 week window after the application gets submitted.

Please verify your level of certifications and send an email if something is missing or listed incorrectly. I’ve listed the highest grade or level of certification achieved.
Also please let me know if you have your CDL or Traffic Control Flagger (TCF) card.
