Please review and provide signatures on the Application and LUCS. Along with the two signed documents the following items are required to be submitted to DEQ: -Full-size (22” x 34”) Erosion & Sediment Control (ESC) Plans -Electronic copies of signed Application and LUCS, and ESC plans -Application fee of $1932.00; make check payable to Oregon DEQ Application materials should be submitted to DEQ at this address: Oregon Dept. of Environmental Quality Northwest Region, Water Quality 700 NE Multnomah Street, Suite 600 Portland, OR 97232 We have a couple ways to handle the submittal to DEQ: 1. Return signed Application, LUCS & application fee (check) to me and MSA will submit the packet to DEQ. 2. I can send the full-size & electronic plans to you and the City can make the submittal.
