The application along with the project’s Erosion and Sediment Control (ESC) plans will need to be submitted for review and approval at DEQ headquarters in Portland. DEQ charges a permit application fee of $1,725.00. Attached is a completed application form for your review and signature. Also attached is a partially competed Land Use Compatibility Statement (LUCS), which is part of the application. Page 2 of the LUCS needs to be completed by the City and signed. On the application the City is listed as the applicant, but the permit will be transferred to the project contractor upon award of contract. This will be specified in the bid documents. After your review of the ESC plans, which are included in the project’s 90% design plans, MSA can make any necessary edits and submit the completed, signed application & LUCS, plans and fee to DEQ, which is located close to MSA’s Portland office. It would be convenient if we can pick up the completed 1200-C application documents and a check for the $1,725.00 fee from you at our upcoming 90% design review meeting on Wednesday the 30th.
