Subject: Molalla 5 - Day report.

Hello Jennifer. Just follow what’s in your permit, as you’ve already outlined, and you’ll be fine, in terms of what you need to address in the 5-day report.

Since the SSO was completely contained on your property and did not impact waters of the state, or otherwise require an emergency response, you did not need to call it in to OERS. So don’t worry about having to generate an ORES incident number. Just concentrate on completing and submitting the 5-day DEQ report. Thank you.

Hi David,

I have a few questions for you. I have read through the protocol in our NPDES for the 5-day report, Pg. 30, Section D5a.(2). I just wanted to make sure the NPDES lists (a thru e) everything the City will need to report?

Also do you have an OERS incident number I can add to the report?
