Thank you for using DEQ’s Circuit Rider services. In order for us to continue to better serve you and others, we ask that you take this brief customer service feedback survey regarding the assistance you received from DEQ in the past six months or so.
Survey Monkey keeps your identity confidential, so unless you would like to identify yourself in the survey, know that your feedback is anonymous. All responses will be assembled and not shared as individual feedback with our Circuit Riders. And of course, your participation in the survey is voluntary.
