Attached is my attempt at completing the SSO report. I did not add the start time or quantities since I am unsure of what is decided to be most accurate. I also did not mention the 5gpm (~1/4 of a hose flow) into Bear Creek witnessed by Jason. I did not see where it requested this info on the form. Probably just need to provide that as an update to the 24 hour notice.
I did my best looking up water sources on the following link, but could not verify any existed on Bear Creek downstream of the WWTF. https://yourwater.oregon.gov/search.htm
