DEQ is in receipt of the Limited Phase II Environmental Site Assessment provided by BB&A Environmental. The research about your property found that there was a former service station located on the property, which dispensed fuel. Soil and groundwater samples were collected and found detections of petroleum constituents in the soil and groundwater; this initiated the release reporting to DEQ. No further documentation has been received.
The presence of these petroleum constituents at your site needs to be evaluated against DEQ’s risk-based decision making table located here:

Please submit a risk-based closure evaluation and/or report so a Project Manager can review and make a determination regarding site closure. Hi Jennifer,
I am a bit confused on why we are required to fill out all these reports. We completed a Phase II for a refinance a couple of years ago, and for one reason or another the company that completed the report told me I needed to submit a LUST filing with DEQ. The site has come back clean with all environmental reports over the years. Can you fill me in on what exactly I am required to do?
