Hi Andy,

I am working on this application and need some additional information in order to complete our review for the proposed activity. Note: This application was assigned the number NWP-2021-328. Please reference that number in future communication.

1. Please provide both cross section and plan view drawings that show the area and depth of proposed removal around the intakes.
2. Additionally, please indicate the location where you are proposing to dispose of the excavated material on the drawings.
3. Have you submitted a certification request to Oregon Department of Environmental Quality for a 401 Water Quality Certification? Please provide documentation if so.
4. Is there a Corps permit associated with the original intake installation? I have not been able to locate one so far. If you have a Corps number ( ex. NWP-XXXX-XXX) associated with the structure please provide that.

In order to complete our review of the application please provide the requested items within 30 days. If this information is not received in the specified timeframe, the application will be withdrawn. If you submit the information at a later date, the application can be reactivated.

Subject: [Non-DoD Source] City of Molalla Joint Permit Application

Good Afternoon,

Attached is the City’s permit application for Water Intake Structure maintenance. This permit goes with an emergency permit granted by DSL on which the City entered the water way on June 4, 2021 to protect the intake from unseasonably low flows. The City will send hard copies along with checks for the application fees

Will you please confirm your agency’s current fee amounts? The link in the form to fee schedules is broken (404 error), so the only one I know is DEQs fee listed, $985. Confirm? I can then issue POs and begin that process.

Michael, DSL’s hard copy will be sent in the mail this week.
