NEW WO#4725 [Utility_Crew]: requested by apeters. Promise Date: 1/11/2019 12:00:00 AM

We have an email agreement dated 12/31/18 with Combined Business Services (Polly Burkoff), See Andy for details. Our part is the following:

1. Replace the sloughed material in the channel that runs along the edge of the new sidewalk, from that edge up to three feet wide. The use topsoil and bark mulch as materials.

2. Do not attempt to change grades or slopes on CBS property.

3. Perform work up to an invoiceable value of $1500, then Closed-Complete this work order. The WO will notify CBS on Closeout.

Informational: In consideration of the work being done for CBS by the City of Molalla for the invoiceable value above, CBS has agrees to indemnify and save harmless The City and/or Landscaper (Utility Crew) from any and all defects appearing or developing in the workmanship or materials performed or furnished either under this contract or past contracts adjacent to or on the real property at 416 N Molalla Ave.
