This is not the original document used when the last permit was developed. This document is based on current factual data and shows the classification based on that current factual data. We will use this support our future work (updating if needed if they makes changes) and it shows that the current plant and collection system are classified correctly and match the classification we used in the permit we issued last year. It is not a recreation of the missing document.

This spring, DEQ's Northwest Region Office will be moving to a new location - the 700 Lloyd Building at 700 NE Multnomah St., Suite #600, Portland, OR 97232. The target date for operating at the new location is May 18^th, 2015.

Tiffany, So this is an after the fact document - not the document that supposedly was used last year to rate Molalla? I find it hard to believe that doing that is permissible? How can it be ethical to insert a document after the fact for a waste water plan that has already been approved based on a classification sheet that you can't find? Susan

Hello Susan

Per your request, I am attaching a new work sheet. As I explained in may April 17^th email, I have looked as have my staff for any copy of the original worksheet used to classify the collection and treatment systems. Very unfortunately, we could not find it. We have updated our classification work sheet format so we used that new format to collect data for the classification determination and created a new document. A copy of that is attached here.
