Thank you for the fee waiver. I believe earlier this month Leela Yellesetty sent you a Word version of the fee waiver request form that allows for easier use. Please submit your waiver request on that form or by completing our online waiver request submittal form using the link below. At this time, we do not know if there will be charges related to your record request. If there are charges, you will receive a fee estimate; that would be the time to submit a request for a fee waiver.

The below PRR was submitted today. Find attached the document filed on March 2, 2018 to document our Bear Creek Recovery request for a fee waiver. The document fails to allow me to change the date of March 2 but all the BCR info remains the same. I have requested and have a word.docx to use for these fee waivers but when it is submitted it changes it so it can't be altered. Please furnish me with a form I can use in the future to save as word and then alter the date for a specific new request so I don't waste my time over and over filling in the same info about why BCR qualifies for a fee waiver.
Again, the only information that needs to be altered on the attached fee waiver is date to be March 23, and info request:
Attn: Tiffany Yelton-Bram: Requesting all records - notes, draft documents, emails, letters, etc.- in possession of Tiffany Yelton-Bram regarding interaction with City of Molalla and/or DEQ staff regarding wastewater compliance, permit changes/amendments, new MAO drafts, facilities upgrades, funding, warning letters, violations, fines, leaking lagoons, etc from March 2, 2018 to through March 23, 2018.
Bear Creek Recovery works for the good of the public and shares information widely and thus as usual requests a fee waiver should any fee be necessary for this request. BCR is filing a fee waiver request form on March 23, 2018.
