I appreciate that Ms. Yellesetty answered her phone and quickly provided this form for future use. Unlike your hideous Adobe form, this form can be saved and easily altered for the next request, since the info except for specific request and date will be the same over and over for my non-profit (again, pointing out the waste of our time having to re-file with every request when when we have been allowed fee waivers for years!). I hope as asked that Ms. Yellesetty will post this easy to use, easy to save, easy to alter word version of the form on the website so all of the interested public can find it and use it instead of the hideous Adobe form.
