Subject: Fee Waiver form for DEQ is NOT user friendly to the public

Dear DEQ,

 

Today I again tried to use your required new fee waiver form which you now demand be filled out every time the public requests a fee waiver. In the past, we were granted a fee waiver that was for a blanket cost and timeframe. Your new form is horrible, I had to call Ms. Yellestetty to figure out how to send it, it is not easy to fill in (thus discouraging the public from using the fee waiver form!). I request that ODEQ quickly add an easy to fill in, easy to save and send word option. The email address to send said form(s) should be readily available on the fee waiver website along with a contact phone number so the public can quickly get help if needed. If one tries to use your horrible Adobe form, it get the user tangled up in Adobe offers to sell upgrades to the user, it makes the user struggle to even figure out how to fill and send the form, it makes the user believe a password/account must be created. We the public are not office forms wizards and we should not in any way have to struggle to request public information or fee waivers from the very agencies we fund!

 

Transparency is lacking enough in our government without ODEQ producing difficult to use and difficult to send forms. Please fix the above mentioned problems so the general public can quickly find, fill and transmit the required forms.
