Name of Citizen: Michael Willcut

Mailing Address: 613 wedgewood dr

E-Mail: Willcutm@gmail.com

Phone Number: 5039830270

Department:

- Street

- Water

Nature of Concern: I believe there is a leak in the water main that runs

down wedgewood dr. Water is seeping up through the pavement in the crown of

the road and draining to the curb in front of 619. This is very close to

the same location as the main break last year. I've noticed the road has

been wet for the last few days.
