The way I made the form, they are supposed to look at the list on the back of the form and fill in what type they are. Does that work?

Darlene the only thing I would add is instructions for them to “circle all that apply” or something like that (unless I missed it?)

This is a very important change, thank you. The sewer rates will eventually be different depending on the category, so if a business self-declares their category up front, then it will solve all kinds of problems. Here are four I can think of just off the top of my head. Thanks for taking the time.

  1. a. finance is not forced to make an arbitrary decision about a business class, which could put us in a bind later if they find out an equivalent business just next door is given a different class!
  2. b. Or if we find they are using sewer differently than they declared, then we will be able to handle that easily.
  3. c. Or if we see the business is obviously something other then what they are declaring up front then we can red flag the whole process right at the start, which will save tons of grief later,
  4. d. or if DEQ decides to crush us because they find someone is making illegal discharges they can’t pin it on the City for not knowing… because the business declared what they were going to do and then did something different!!
