Hi Ginger,

We don’t create that form in Public Works. Attached are our fees per Resolution 2018-20.

Not sure where that form came from. May have been a left over from Aldo and I'm not aware of Brenda doing one but anything could have happened with her.

The Planning Dept says you generate this form? If so, can you please change the Utility Deposit fee to $100? Or, is there a reason you're charging $125 that I should know about?
