You would fill out this form and first submit it to your department head for approval and then the department head would submit it to me for approval at which time I will give it a number and determine disposal or archive.
The OAR number for those types of records is 166-200-0385 (23) and (25) in the description area put what the report is and the years span it cover for example
(Turbidity Reports 1970-1974 YRs )

Kelly ,
We are trying to make some room in our file cabinets and would like to be able to purge some records. We are looking to purge some turbidity charts that are over 10 years old and some plant turbidity and chemical dosing worksheets that are 10+ years old. What is the proper way to accomplish this? I think we pretty much have every record since the day the treatment plant was installed and I don’t see a need to keep such information. Let us know. Thank you.
