Subject: New Leave Request Forms for PW
All,
Attached is our new leave request form for Public Works. A similar form will be rolled out for other Departments within the City at a future date. For our staff, I wanted an electronic copy that everyone has access to so I am emailing this out to you for your use. Starting tomorrow please use only this form and submit your time off request at least two weeks in advance. Sick, OFLA/FMLA, Bereavement leaves are obviously exempt from the two week advance notice. This way we can plan workloads as the City forces grow in size and complexity.
There is a section pertaining to coverage. The Maintenance Division will need to maintain a set number of staff on duty on Friday’s. For now, that number is two at a minimum to address issues that may come up and is subject to change on a case-by-case basis depending on City operational needs. Essentially, nothing has changed in the availability of time off. The “Other” types of leave can be OFLA/FMLA, Unpaid, and any other leave that may happen in the future for flexibility of the document. Thanks and let me know if you have any questions.
