Hi,

The system actually tells you when you elect what the cost will be, and you would have had to complete an additional personal health application to receive this additional amount, which also told you the cost. Rates change every 5 years (40, 45, 50, etc). That said, I will remove this per your request and drop it down to the $100,000 and $20,0oo respectively.

Hi Heather,

I was never provided any information on the cost per thousand additional and don’t know when the rates change, whether yearly/5 yrs/ 10 yrs. Also, the website that I signed into wouldn’t update and our HR folks couldn’t get it to work correctly either. That being said, I want the insurance to return to $100,000 for me and $20,000 for my wife and the City to be credited for any charges outside of that. I shouldn’t be a problem since we are both still alive and no claims have been filed since startup. Thanks Heather.

Gerald applied for additional supplemental life insurance that was approved effective September 1. He was paying for the guarantee issue amount ($100,000 for him and $20,000 for his spouse), and now he is paying for the total amount he applied for ($270,000 each) – see below.

I looked at the August billing for Gerald Fishers life insurance and the rates were 18.50 and 2.72 Now September is 49.95 and 36.72. Gerald was going to call but has no clue who to talk to so we are asking for some information.
