Hello Andy,

COVID-19 Testing Available in Woodburn at Legacy Woodburn Health Center drive-thru

If you or your employee suspect they may have COVID-19, Legacy Health has several ways to assist in assessing your employees for illness and/or return to work.

1) The Legacy Health COVID-19 Assessment Tool (available in English and Spanish) located on our web site at https://legacy-c19.smartexam.com/patient/covidregister/ can be used by your employees who have symptoms or exposure to COVID-19 free of charge.

2) Legacy Medical Group Primary Care patients may choose to contact their Primary Care Provider (PCP). Most PCPs are doing telephone and video visits at this time. Our PCPs can evaluate their patients, assist with managing symptoms and refer for testing as necessary.

3) Business Health Services (BHS) is providing telephone visits to all businesses. Our providers can assist businesses whose employees may be exposed at work or need guidance for return to work. Business Health Services can also refer employees for COVID-19 testing. All visits with Business Health are billed to the employer.

NOTE: This service is available to ANY business, not just those that are clients of the BHS clinic. If you know of other businesses who can benefit from this service, please share this email.

Steps to make an appointment for a telephone visit with Business Health Services are as follows:

  1. A. Have an authorized company representative send an email to the clinic at BHS@LHS.org with the name(s) of any employee who is authorized to receive a telephone visit. Be sure to include:

 

i. Company Name (and parent company name if applicable).

ii. Name of all employee(s) authorized to make appointments.

iii. Billing address.

iv. Authorized company representative contact information.

  1. B. Have the employee on the list call the BHS clinic at 971.983.5340 to set an appointment for a telephone visit with a BHS provider. We must have your authorization in the form of the email mentioned in step A prior to the employee calling. Our provider will assess the patient’s symptoms and exposure and advise whether COVID-19 testing is warranted. Please Note: We are not “blanket testing” everyone.

 

i. If the BHS provider determines testing is warranted, the patient will be contacted by a Legacy testing site staff member to set up their drive thru appointment at Legacy Woodburn Health Center, 1475 Mt. Hood Avenue in Woodburn.

Following drive thru testing, both the company (via authorized company rep. listed in step A) iv. above) and the employee will receive results usually within 1 day, but no later than 48 hours. The patient and the company will receive guidance for return to work following testing.

ii. If testing is NOT warranted, the employee will receive a Return to Work note to present to their employer.

PLEASE NOTE: All fees will be billed to the company. Charges include a phone consultation charge that will vary depending on complexity (i.e. length) of the call ($40-$125 range) and, if warranted, a COVID-19 Test fee of $60. There will be a $45 no-show fee for non-attendance of the phone conference at the appointed time. Prices are subject to change.
