For all resource requests, please carefully review the attached EOC Resource Request Instructions.
For urgent requests for N95 masks, face shields, procedure masks, gowns, and gloves, complete the Clackamas County EOC Inventory Resource Request Form.
For all other requests, complete the ICS 213RR Resource Request Form.
The Logistics team are working as quickly as possible to prioritize and fulfill requests. Please make sure all email requests and subsequent communication includes the EOCLogistics@clackamas.us email address.
