Dear Grant Manager,

Your organization was identified by the Oregon Legislature in the Budget Report for House Bill 5006 (2021) as a recipient of federal Coronavirus State Fiscal Recovery Funding (CSFRF). Based on information provided to the Department of Administrative Services in the submitted Project Information Form (PIF), we have drafted the attached CSFRF grant agreement.

Please review the grant agreement and complete the following actions:

  1. 1. Exhibit A: Please review the Use of Funds/Project Description: this is a summary of the draft statement of work provided by your organization in the PIF.
    1. a. Verify the recipient name and contact information is correct. Please update any information that is incorrect.
    2. b. If this summary does not accurately depict the activities you intend to complete using the funds provided by this grant, please email us at StateFiscal.RecoveryFund@das.Oregon.gov so we may more accurately capture the project description.
    3. c. The activities described in this section of the agreement will need to be completed before the grant completion deadline of June 30, 2024.
  2. 2. Exhibit C:
    1. a. Please verify the information in items (i) and (ii) of Exhibit C are correct. If the Subrecipient’s Unique Entity Identifier (UEI) is blank you will need to provide either the UEI or the DUNS number when returning the completed agreement.
    2. b. Please also verify the award amount in (vii). If it is incorrect, please email us at StateFiscal.RecoveryFund@das.Oregon.gov so we may accurately provide the award amount.
    3. c. Please verify information in item (xiv). If your organization will be using a negotiated indirect rate (your accounting office will know if you have one), please indicate the rate in subitem a., and in subitem b., please uncheck “Yes” and check “No”.
  3. 3. Finally, please obtain the authorized signature and date, in the space below the entity name at the end of Section 10 on page 8. Please also complete the name and title of the authorized representative. Note: The authorized representative is the person in your organization with the authority to enter into a legal agreement on behalf of the organization.
  4. 4. Once the agreement has been reviewed and signed, you may return the completed agreement to DAS by emailing StateFiscal.RecoveryFund@das.Oregon.gov.
