Importance: Normal
Attachments: LUST Cost Recovery Agreement Form.pdf; InfoPacketLUST.pdf; Good afternoon Mr. Gregg, Rob called and let me know you sent a $5000.00 deposit check for the above mentioned site. I’m working with DEQ Salem office to catch and cancel the check. A $5000.00 deposit check is required for the Voluntary Cleanup Pathway (VCP) program where we work closely with the responsible party and consultants developing a workplan, site investigation, and clean-up, if needed. This site will go through our LUST program where we will need a signed cost recovery agreement (CRA) from the responsible party in order to get you on the waiting list for a project manager (PM) to work with you. You will be invoiced as the PM conducts review, consults with a lead worker, fills data gaps, develops a staff memo (if needed), and then a no further action (NFA) determination letter, if the site is ready for that determination. Once you return the signed CRA (via email to Dylan) DEQ will determine the complexity and priority of the site. We also like to know if the property is going to be sold or refinanced in the near future, this will make the site a higher priority. After the determination is made your site will be placed on a waiting list for a PM to be assigned. During this waiting period Dylan Whitney will be your intake coordinator and will work with you gathering any pertinent information to help the PM get up to speed quickly on the project. Looks like we have a Phase I report from 5/5/2005 and a Level I & II risk assessment from 5/30/1990. Usually you would need to provide a Twenty Day Report, since this is an older release we may just need the CRA at this point. There is also a cleanup report that is typically due 45 days after the date of the release. Lastly, DEQ will need a Final Cleanup Report. The final report must include information outlined in the UST Cleanup Manual, OAR 340-122-360. https://www.oregon.gov/deq/FilterDocs/USTCleanupManual.pdf Summary of Documents:
* May not need these.
As intake coordinators we can’t get too deep researching the sites to offer you site specific guidance.
Dylan will keep you updated on where the site is at in the waiting list queue and once he knows when a PM is assigned.
You will also receive a hard copy letter announcing the PM.
