He needs a permit before he can start work. Please have Brenda sign the local agency box. Then have the contractor sign both the preliminary permit and the general provisions on each page where it says “Applicant signature. See attached. PDF back to me once you get all of the signatures. Then have the contractor provide the $2000 performance bond and the insurance that makes ODOT additionally insured and the certificate holder.

The contractor is Jim Knapp, Knapp Construction Co, Inc. cbc# 38508. Yes he's insured and bonded and has done work for Cities of Gladstone and Milwaukie. He said specs are 3-4 inches of compacted gravel and concrete at 4000 psi. Before hand he will rip up old concrete and dig out tree roots. He has ordered cement to be poured on Monday. Do frames need inspected before concrete is poured?

Please tell me how to apply for a permit to replace a section of sidewalk along highway 211. Exact address is 524 E Main St, Molalla, Or 97038. City of Molalla requires sidewalk replacement before a partition can be approved.
