The uninspected patch will be t-cut 1 foot per our detail drawing, removed, and inspection will be required prior to and during paving. Traffic control is required as well as compaction testing of pavement. Minimum thickness of patch will be 4 inches or match existing whichever is greater.

Jacques,

You recently paved back the trench line on Shirley Street but failed to call for an inspection before placing HMAC. You need to remove this patch and redo. Please call for directions or read the directions on your permit which state you must call for inspection. I have no way of knowing how deep this patch is or if you did a T-cut or not. This is not acceptable.
