I checked on the need for this form to be completed and sent to DEQ. Rather than August 18, it will work for us if your office could have the completed form emailed to Shelley Matthews in AQ by Friday 8/22 or by the early morning of Monday 8/25 at the latest.

I double checked, and it is just the one form (attached) that we are asking be completed. My apologies for not making this request earlier. I would guess that much of the needed information could be copy and pasted out of the "LRAPA Board of Directors Agenda Item Summary" from the March 14, 2008 meeting. I have attached an example of a completed version of this form to give you some sense of what a completed form looks like. Please call Shelley or me if you need assistance addressing the topics within this form.
