I made checkboxes instead of dropdowns because there were only a few options and sometimes it better to see what options are available up front, but I can change them to dropdowns if you prefer.

You can put whatever URL in the DocLink field. The ones in the fileshare will be \\deqhq1\fileshare\filename and the ones to the SOS document will be http://www.sos.state.or.us/...
