DEQ submitted the DAS fee approval documents to DAS on 6/19/2008 (signed by Kerri Nelson for Joni Hammond). Then about July 10^th, Larry checked with Lisa Pearson at DAS for an update on the status of those fee approval docs – only to find out that DAS said they never received them. DEQ re-sent those docs on 7/11.

We have copies of the fee approval forms and cover letter sent on 6/19. I am confident that the docs were mailed to DAS on 6/19. Interesting that we have had other similar experiences with DAS indicating they never received our documents.
